Completing an accident investigation



4 steps to conduct the accident / incident investigation

The on site Lead must complete the investigation and fill out the report within 24 hours of the accident.

After the injured Associates receives medical care, make sure they report the claim to MEDCOR at 1-(800) 775-5866. If the Associate is incapacitated, you must make the call on their behalf. Do this as soon as you can. Then, start your investigation. An Incident Investigation Report can help you keep track of your findings. Follow these steps to get started. The Supervisor investigation form is in two places. The first is a printable PDF and the other is an electronic form. SASR PREFERS THE ELECTRONIC FORM. However, if you need to use the paper form complete it and email a photo of the report to safety@sasrlink.com.

Do the following: 
1. Secure the area
Secure the area where the incident took place with tape or cones. Direct Associates away from the area. Were any machines or equipment damaged? Are there hazards in the walkway? Use your smartphone or a camera to take pictures of what you see. Upload pictures in the spaces provided on the electronic investigation form. 

2. Get witness statements
Timely reporting and investigation are important. You want to talk to Associates while memories are fresh. Have anyone present during the incident fill out a Statement. Take photos of the statements and attach into the investigation form. 

3. Analyze your findings
What did you find in your investigation? What changes could you make to help keep Associates safe? Was an Associate at fault, and is corrective action needed? Remember, your goal is to keep the incident/accident from happening again.

4. Identify specific actions
Identify the specific actions you need to take to make your workplace safer. Assign those responsibilities to Leads or Associates to make sure they get done. If Associates need training on safety rules, set aside time to do that.
